Can you tell how can I add a fourth field to the contact form.

I need a basic field that the customer can populate.

example: domain name: ........
the second question, the field headers, where can they be modified. I have been looking around with no success, I may have overlooked the fields.

you have to edit contact.tpl and sendmail.php.
