Categories behaving oddly

This is really strange. I have 3 categories (General, Templates and Documents). When I look at Categories in the admin panel, I see the 3 categories like they are supposed to be. But if I go under FAQ Administration, they show as "General/General", "General/Templates" and "General/Documents". But no category is set to be a sub-category of any other category. And when you go to the client side and view the FAQ, the breadcrumb shows as "General / General" respectively.

And to make things even stranger. If I change the order of the categories, whichever category is at the top becomes the first part of that name. So if I move Templates to the top of the order, the categories now show as "Templates/Templates", Templates/General" and "Templates/Documents".

For now, in my site template, I have hidden the Breadcrumbs section to avoid confusion. But would like to figure out why it is doing this.
