Fresh Install 4.0.1 Issues

Hello,
I'm doing a fresh install of 4.0.1 but after completing the admin portion during Setup, I hit submit and get a message that the requested URL was not found on this server. It redirects to 'https://mydomain.com/setup/install'

I'm confident folder permissions are correct but there is definitely not an 'install' folder inside of the '/var/www/mydomain.com/setup' and creating one doesnt seem to make a difference.

your rewriting does not work correctly. Do you use Apache or nginx?

Using Apache, I have mod_rewrite, mod_ssl, and mod_headers enabled. AllowOverride All is also set and site is installed into the root folder at /var/www/faq.mysite.com
