I still have this problem with version 2.5.2 (2009-09-01).
When I ask a question which is related to another one, I am asked if I want it to be saved, when I confirm the question any new question is stored.

we cannot reproduce the problem... what exactly is going wrong?

I tested it by creating a new FAQ entry in category1:

Question: "attention caution?"
Answer: "phpMyFAQ"

Then I opened "ask question" and wrote "attention caution?" in category2.
After submitting you will be shown the related FAQ entry and you will be asked if you want to save your question anyway.
When I confirm "No, there is still no adequate answer (will send the mail)" there is not saved a new open question (either in the admin panel).

When the related entries are in the same category sometimes a new open question is being saved.
